About the Position
Graham Windham seeks an experienced Nurse Practitioner to provide quality medical care to the children and adolescents placed in the agency’s care.

Specific Responsibilities:

  • Ensures that medical services are provided per mandated timelines, records are kept up to date per mandated standards and the program is ready for internal and external audits (Including Medical review by ACS; PES/EQUIP medical review by ACS and COA review).
  • In collaboration with the campus health service team, develops and implements a health education curriculum and training program, focusing on human development, preventive medical care, hygiene, AIDs/HIV and family planning/pregnancy prevention.
  • Demonstrates increasing fidelity to Solutions Based Casework and Collaborative Problem Solving (CPS). Attends and is an engaged participant in all scheduled trainings and coaching sessions. Works from a “kids do well if they can” frame of reference and proactively engages with youth and adults in problem solving discussions.
  • Obtains and maintains certification as an OSHA trainer; provides annual OSHA training to Westchester based agency employees.
  • Provides primary medical care to the children and adolescents of the Westchester based programs. This includes, but is not limited to physical examinations, assessments, diagnostic and therapeutic procedures; communicating with community-based medical professionals, families and staff members of other disciplines to ensure ongoing quality delivery of health services.
  • Provides medical care according to standards set by OCFS/ACS, including health education, HIV screening, counseling and treatment.
  • Collaborates with the contracted physician on site and/or communicates with medical professionals in the community as necessary in managing care for the children; reviews past medical histories and immunization records; writes treatment plans.
  • Monitors medication storage and handling.
  • Oversees the ordering of immunizations and ensures compliance with DOH standards.
  • Evaluates and assesses injuries for state investigated cases of alleged abuse; provides reports when requested, including photo documentation when necessary.
  • Refers children for consultations and follows up as necessary. Monitors the children’s complimentary services (i.e. medical, dental, hearing, endocrinology, etc.) provided by community-based hospitals/clinics/agencies to ensure quality services are received; including review of written feedback provided post-appointments.
  • Reviews monthly orders for prescriptions& reorders as needed.
  • Collaborates with the various members of the youth’s interdisciplinary team (i.e. permanency planning, residential, clinical, administration, the school) and is an active participant in promoting a youth’s health and wellness.

Minimum Qualifications

Licensed Nurse Practitioner; Master’s of science degree in nursing; (MSN) is preferred; Minimum of five (5) years’ experience working with children especially adolescents; Valid CPR certification; Valid American Red Cross First Aid Certification; Valid state driver’s license; NYS prescriptive rights.
